Park Leys Medical Practice

Park Leys Medical Practice welcomes new patients who live within the boundaries of the practice area.

If you wish to register with the practice please complete the online registration form, or submit a completed GMS1 form to one of our reception areas.

You will also be asked to submit some additional details regarding your medical history, which can also be done on line and invited to attend a New Patient interview with a Practice Nurse or Healthcare Assistant.
