General Practice Assessment Questionnaire (GPAQ).

The General Practice Assessment Questionnaire (GPAQ) is a patient questionnaire which has been developed at the National Primary Care Research and Development Centre at The University of Manchester for the 2003 GP contract. Building on several years of development and testing, GPAQ helps practices find out what patients think about their care. It specifically focuses on aspects of general practice that are not covered elsewhere in the Quality and Outcomes Framework - for example, access, inter-personal aspects of care and continuity of care.

In order to meet the requiremenns of QOF, the practice must complete a survey each year, with 25 forms being submitted for each 1000 registered patients. The practice must consider the results of the survey and develop and action plan which must be discussed with the Patient Forum.

Every year the practice asks nearly 300 patients to complete the GPAQ survey form and are grateful to those of you who have taken the time and trouble to do so, and also to members of the Patient Forum who give up their free time to help the practice develop the sevices it provides to patients and to help us meet the requirements of QOF.
