Patient Forum
Come and Join Us
by Amelia Freck, Chair, Park Leys Patient Forum.
In October 2006 the Patient Forum was formed after doctors and staff from the surgery approached patients that they thought would be interested in proactively working with the surgery to offer valid constructive view points.
The first meeting was conducted on 19th October 2006 and roles were identified. This is a major step in forming relationships with patients, to enable the local community to have some valid input into raising concerns, or worries, regarding various matters and issues that impact on patients. Presently this is a small group of patients who meet with doctors and members of practice staff, with a view to promoting good communications and working in partnership with the practice to develop the services that the surgeries provide in line with patient expectations. The Patient Forum would warmly welcome patients, willing to attend meetings, to represent their community, families and friends, and make a constructive contribution to the Forum and the Practice.
The dates of the next meetings are listed below and on the notice boards in the surgeries. If you would like to participate in the meetings or bring an issue to the attention of the forum please fill in a form (available from each Reception) and post in one of the boxes provided. A member of the forum will contact you following the next meeting, which are held about 6 times per year. You can also contact the Forum by email, by following the link below. Someone from the forum will contact you.
At an early meeting of the forum, a vision statement was agreed and reproduced here. If you would like to make a positive contribution to this group please do not hesitate to get in touch and help shape the future of your surgery.
Best Wishes
Amelia
